How to select someone to be your voice (agent) when you can no longer speak for yourself and the forms needed to make sure that is honored. This is a values-based decision process and I will help you determine the values that will affect your choices. I will also help facilitate conversation with you and your selected agent.  Rate: $150 for (2) 1-hour meetings (additional hours will be charged at $50/hour)

Identifying through conversation and a checklist, the documents (i.e. life insurance, banking, and bills) and arrangements (i.e. funeral) that would be beneficial for your loved ones to follow when you depart. This would reduce stress on loved ones during an emotional time. Assisting with providing needed resources of referrals (See Referral/Resources). Rate: $200 (2.5 hours)

From diagnosis to transition there is a lot of unknown and questions that come with that. I can assist you with developing personal plans to coordinate services, provide you with education/information, or create a list of questions for your providers. Rate: $50/hour (2-hour minimum)

Assist with creating a flexible plan that will list how you would like to arrange your space (consisting of sights, sounds, smells, touch etc.) how you would like to interact/not interact with loved ones as you are actively approaching end-of-life. This assist loved ones in understanding and able to support what you want done, not leaving them to guess. Rate: $50/hour (2-hour minimum)

Helping to facilitate end-of- life conversations with clients and loved ones. Advocating for the client wishes to be heard and supporting with them being honored. Creating space to respectfully share emotions and concerns related to clients end-of-life journey. Rate: $125 for a 2-hour session (additional hours will be charged at $50/hour)

Gives you the ability to tell your story in your own words. What a wonderful way for your loved ones to have a memory of you by you! Written/digital (audio recording flash drive included) sharing of stories, letters, lovingly advice, journaling, or other creative ways of remembrance.

Using an obituary template, you will get assistance completing those details of your life that can be difficult for loved ones to remember during such an emotional time. This is also a perfect opportunity for reflection. Rate: $125 (2 hours)

Grief comes in many forms. Loss of a loved one in different forms not limited to older or youth; but including abortion, perinatal loss, or infant loss. Sometimes we need outside support to help with the transition.
You will receive non-judgmental support and resources. Includes in-person or phone communication, scheduled phone/video call (up to 60 minutes), unlimited text/messages, emotional support, and resources.

Sliding Scale Service

This service is included with End-of-Life Planning but can be an individual service. You may have requests for services not currently provided by Dignity With Departure or need additional information regarding your circumstances. The goal is to help close the gap and alleviate stress by searching for and providing referrals or resources to address your needs. This includes a one-hour meeting to determine what those needs are and a follow up meeting to provide the information.  These may come with additional charges that are separate from Dignity With Departure’s rates. Rate: $100

Being present for departing loved one requires time and energy. You also need time to take care of yourself. 2-hour blocks of time provided devoted to your departing loved one, so you can go to grab a bite to eat, watch a movie, visit friends, go for a walk, or even take a nap! Know that your loved one is being attended to in a respectful caring way. Rate: $50/hour (2-hour minimum)

Scheduled visits per request. Rate: $40/hour

In-person doesn't work. You can schedule a phone per request. Rate: $35/ up to one-hour
